Acting on behalf of my minor child, I do hereby waive and release “Hollywood Kids Academy for the Performing Arts” and all it’s respective officers teachers, assistants, employees, sub-contractors, and/or representatives, from any and all liability for personal injuries or damages sustained, incurred from any and all liability for personal injuries or damages sustained, incurred, or arising from participating in “Hollywood Kids Academy for the Performing Arts”.
I give my permission for my child to participate in field trips when offered and do hereby authorize the adults supervising the activity to obtain emergency treatment for my child if necessary and to act in my stead in approving necessary medical care. This authorization shall cover the activity and travel to and from the activity.
I give my permission for Hollywood Kids to use any photographs that may be taken of my child for advertisement purposes. No personal information including names will be used unless pre-approved by parents.
